In addition to serving our clients, Care 4 America always encourages the
family’s involvement.
Care 4 America helps seniors with care management and mental health issues in the privacy and convenience of their home, nursing home or assisted living facility. Seniors are also welcomed to enjoy their counseling at the Care 4 America Family Counseling Center in Gainesville.
Our Licensed Clinical Senior Care Social Workers help seniors to enjoy life! They address mental health, anger, depression, loneliness, memory issues and everyday living issues such as family and other relationships, safety, good ways to spend time, and money management.
Care 4 America Geriatric Care Management is either short-term or on-going assistance for long distance caregivers or others requiring assistance. Care 4 America offers personalized and compassionate services focusing on the individual's wants and needs. We provide continuity of care by facilitating family communication, reducing the burden on families and preventing unnecessary expenditures.
Geriatric Care Management is a fee for service, usually facilitated through insurance. Seniors are provided with a licensed professional clinician who assists them with community referrals, case management, co-ordination of physician appointments, and coordination with children who cannot visit frequently. Home safety is assessed on every visit. Important basic needs such as nutrition, taking medications properly and organizing medications systematically are also discussed.
We accept most insurance. In the event that your loved one is uninsured, we utilize a sliding scale that is based on several factors and income to compute. When you visit our office for the first time, our business office personnel will discuss and compute your affordable fee. The Care 4 America service area is in central and northern Florida and is described below. Our main office is in Gainesville.
